 REQUIREMENT TO PROVIDE PRELIMINARY AND FINAL SITE
 PLANS, AS REQUIRED BY THE LDR’S, FOR THE ABOVE SUBJECT
 PROJECT. YOUR REQUEST IS CONDITIONALLY-APPROVED AS
 MEETING THE REQUIRED FINDINGS FOR AN EXEMPTION. OUR
 OFFICE HAS ALSO REVIEWED THE PROPOSED SITE PLAN AND
 SUPPORTING DOCUMENTS AND FINDS THAT THE SITE IS IN
 COMPLIANCE WITH SECTION 12.04.215 ANTENNAS AND GROUND
 MOUNTED TOWERS OF THE LDR’S.
 PLEASE BE ADVISED THAT THE FOLLOWING INFORMATION MUST
 BE SUBMITTED TO OUR OFFICE PRIOR TO THE BUILDING
 DEPARTMENT ISSUING THE CERTIFICATE OF COMPLETION:
